This page provides insight reports on Surrey’s NHS Place-based partnerships.

There are 7 NHS Places that overlap Surrey county boundaries. There are four place-based partnerships within Surrey Heartlands ICS, and all four of the place-based partnerships in Surrey Heartlands ICS sit within Surrey county boundaries. Three of the five place-based partnerships in Frimley Health and Care ICS overlap with Surrey county boundaries: all of Surrey Heath, part of North East Hampshire and Farnham and a small part of Windsor and Maidenhead. A Place Pack has been produced for 6 NHS Places which contains demographic information and wider determinants of health indicators. Due to data restrictions a Place Pack has not been produced for Windsor and Maidenhead.
